Chronic Disease Management Network
The Chronic Disease Management Network (cdmNet) is an online productivity tool for GPs, their practice nurses, care providers and patients, to assist them to better manage chronic disease. It automates the complex administrative and communication tasks associated with chronic disease management, freeing up GP time and improving productivity. Care Plan Filtering: The Planning section of a patient’s health record now has a filter menu at the top, similar to what you get on the patient list. Using this menu, you can filter your view of the care plan so you only see tasks assigned to you, your organisation, the primary care provider, other care team members, or the patient. The default filter, showing tasks assigned to everyone, is the same as the view of the care plan in previous releases of cdmNet.
Your filter selection stays in place while you’re viewing that patient’s record (including when you move to other sections of the record); when you switch to another patient, or log out, the filter resets to its default selection (showing the entire care plan).
